Explanatory Note: After the Miami workshop (Preventive Medicine 2007) JLN added an "opening monologue" to each of the PSTK presentations to better enable workshop participants to "center" on the topic we will be discussing, why we will be discussing it, and why it is of relevance to them. Since then, with considerable success, JLN has used some variant of this monologue with each of presentation.
